DEFINITION:

Cleaning, examining, documenting, and reporting as necessary, any change   in the catheter exit site and surrounding skin condition.

EQUIPMENT/ MATERIAL:

2 - Surgical masks                                             2 - Sterile towels

1 Pair  Non-sterile gloves                                  4 - 4x4 Gauze sponges

1 Pair  Sterile gloves small/large Adhesive Surgical dressings (Mepore)

Povidone-iodine solution

1 inch tape

Alcohol Swabs

COMMENTS:

1.  The hemodialysis nurse will provide catheter care pre or post

     hemodialysis treatment on the day of dialysis in Hemodialysis Unit.

2.  On non-dialysis days, the nurse on the floor is responsible for providing

     catheter care to in-patients if needed.

PROCEDURE                                                                                           EXPLANATION

1. Put a mask on and wash hands thoroughly.                                       1. Prevent infection.

2. Prepare sterile field with towel and on it

    catheter care.

5. When skin is dry, apply sterile adhesive

6. Document procedure performed and any

    changes observed. Any change requiring
