Scope of Service:
The Renal Transplant Clinics is
a hospital-based Transplant center providing comprehensive nursing care for
adult and pediatric patients requiring Renal Transplant as a treatment modality
for End Stage Renal Disease. Nursing care is provided in two clinics-Pre and
Post Transplant.

nurse coordinators coverage: 24 hrs a day, seven days a week
Medication Refills:
Saturday -
Wednesday: 0800 - 1600 hrs
Patient will call early in the
morning for medication refill and to pick-up the prescription at 1600
Criteria for Admission of Service:
Patients needing transplants
must be referred by other health care providers and are initially evaluated and
approved for tertiary care through Medical Eligibility or Royal Order.

In the Pre Renal Transplant Clinic, each nurse works independently utilizing
the primary nursing care process. The nurse,
in collaboration with the Renal
Transplant Program team, is responsible for coordinating the patient’s work-up.
Potential
recipients and donors are assessed for their suitability for transplant/donation
and followed by the primary nurse until admission for surgery.
In the Post Renal Transplant
Clinic, each nurse works in their respective assignment utilizing the primary
nursing care process. The nurse assign is responsible to the Nephrology/Combined
Post Renal Transplant Surgical Clinics, biopsies, triaging walk-in patients.
The Renal Nurse Transplant
Coordinator’s are responsible for coordinating cadaveric and living related
renal transplants. They perform in the role of consultants to the
multidisciplinary team during the patient’s treatment.
The Renal Nurse Clinical
Coordinator is responsible to perform education to patient and facilitate
clearances of work-up patients.
Method of Adjusting to
Staffing Variances:
The Head Nurse makes
adjustments in staffing based on the volume of patients/procedures, skill level
of staff and number of bilingual or bilingual physicians attending the clinic.
When additional staff is needed, adjustments in assignments are made from
different units within Hemodialysis and peritoneal
dialysis clinic staff that are crossed trained.

Staff meetings are held monthly
and additional meetings are scheduled as needed. Staff development in-services
are posted on the bulletin board to disseminate information. New/review of
policy and procedures are in-serviced by the clinician.

Internally, direct verbal
communication between physicians and other team members occurs throughout the
day. Bulletin boards, an APP and Management Notifications book and Minutes of
the meetings book are used for written communication.
Externally, the clinic
communicates with nursing units and multiple departments via telephone,
facsimile, and written memos.

Program solicits and initiates interdisciplinary practice via

nursing, dietary, educator and social worker attend both meetings.

Transplant participates in clinical pathway development with personnel
from other departments.
