CERTIFICATE OF ATTENDANCE:

Certificate will of Attendance will be available on the final day of the symposium ONLY. Please collect your certificate from the Symposium Registration Desk before you leave, as they will NOT
be available after the symposium is over.
AACME CERTIFICATE
We will airmail it to you within two months of the symposium.
To get the AACME certificate the following procedure must be adopted:
The AACME Attended Lectures Form includes a list of the lectures and the number of CME hours granted per lecture. It is your responsibility to put a check mark next to the activities that you attended.
At the end of the symposium, please return the AACME Certificate Instruction Form with the AACME Attended Lectures Form to the AACME Desk, complete with check marks next to the activities that you attend.
If you do not return the completed AACME Attended Lectures Form to the AACME Desk, you will not receive an AACME certificate.
